Hello.
I am looking for a way to track safety and health issues, both active and resolved covering ten different facilities across the province. I am thinking it would be in either InfoPath or Word format, possibly Excel.
Does anyone have an example they may be willing to share, or suggestions on how such a document would look like?
What I hope to accomplish is to be able, at a glance, to see if similar problems exist at other facilities either currently or previously.

Software like that would be great to have.
There is commercial software I'm sure that would do it, however I am sure it is incredibly expensive.
You could have a access database that would house the data.
Store that on a sharepoint server
Create Infopath forms that would get filled out by the supervisor of each location for each concern/issue/incident, and send to the database on the sharepoint server.
You could have Access generate reports however you wanted them.
The main Achilles heel is that the data would have to be entered in a uniform way.
For example: If there was no standardization. Each supervisor could enter information about an identical issue a different way. This would not be searchable or easily reportable because there would be no common denominator in the data.
If you have a company wide loss causation model, where each issue would be investigated as per their training, then there would be pick lists and/or similar descriptions that would limit the amount of variance in the issue being input.
That may sound like gibberish... Sorry, trying to multitask here.
You may not even want to use the infopath portion, because you can set up forms in Access, however the ease of an infopath form vs giving the report submitter access to the access database may be worth the extra work to set up infopath.
This could be tailored pretty much however you want. The software is there, and fairly accessible. The office 2007 suite makes for some incredible integration of data.
Note: I do not work for Microsoft, I am just learning to use it.
